Former Students:
There are several ways to obtain a copy of your Santa Monica High School Transcript or health record. Choose one of the following:
Requests from employment agencies or other companies requesting a third party verification of graduation or attendance cannot be conducted by phone. Please email your request to samohirecords@smmusd.org. Be sure to include the student's full name when attending Santa Monica High School, complete date of birth, and signed authorization for release of information. Verification requests for students who attended or graduated Samohi from 2001 to present may take up to three to five business days to complete. For students who graduated or attended prior to 2001, requests may take longer.
Santa Monica High School does not keep copies of student diplomas. A replacement diploma may be ordered for $50.00 and will take approximately six weeks to receive.
To order a replacement diploma, please print and fill out the Record Request Form. Mail the completed form along with payment (see payment and mailing information below) and a copy of your current photo ID, to the mailing address below.
